Therapist - Physical PRN

Job Summary This role involves evaluating and assessing patients to establish functional, measurable goals aimed at restoring abilities. The therapist develops and implements a written plan of care, providing treatment aligned with established goals, and reassesses patient progress regularly. They communicate findings, coordinate discharge planning including patient education and referrals, and document all interventions comprehensively. The position may include supervising clinical students and support staff, offering leadership and participating in performance improvement initiatives. The therapist contributes to healthcare quality improvement and serves as a resource for staff and management. Responsibilities Assess patients based on subjective and objective data.Create and follow a written plan of care.Administer treatments according to goals and evaluate their effectiveness.Reassess patient progress and modify care plans accordingly.Provide discharge planning, patient education, and community resource referrals.Support staff and student development.Participate in healthcare improvement activities.Document interventions accurately in medical records.Achieve assigned clinical goals. Qualifications Minimum three years of clinical experience or advanced training with certification in primary practice area.Basic computer literacy skills.BLS certification within 14 days of hire preferred.
