Responsibilities

• Evaluates and assesses the patient based on subjective and objective information.
• Develops a written plan of care.
• Provides treatment according to established goals on the plan of care.
• Documents interventions/care in the patient's medical record.
• Evaluates effectiveness of the plan of care while also reassessing patient's functional status.
• Provides discharge planning information which includes patient/family education, follow-up, and referral to community resources.
• Contributes to the development of staff and students.
• Participates in activities designed to improve healthcare delivery.
• Completes assigned goals.

• Experience: Minimum required, preferred/desired.
• Education: Preferred/required.
• Training: Preferred/required.
• Special Skills: Minimum required basic skills such as keyboarding, using a mouse, and navigating sites. Preferred/required skills include BLS certification within 14 days of hire; PT.
