Physical Therapist Assistant
Job Summary
The Physical Therapist Assistant provides interventions as part of the established PT plan of care, collaborates with PT regarding patient progress and treatments, and educates participants and/or family in care as part of Healthy Aging and Wellness Program. Our community health based model allows our therapists to work in the PACE outpatient center and in participants’ homes as needed. This role also implements plan of care with input from PT as needed, regularly informs the interdisciplinary team (IDT) of the medical, functional, and psychosocial condition of participants, remains alert to pertinent input from other team members and participant caregivers, and documents changes in a participant’s medical record consistent with documentation policies.
Education
- Graduate of physical therapy assistant program with curriculum approved by the American Physical Therapy Association.
Experience
- One (1) year experience working with the elderly preferred.
Other Requirements
- Active State of Washington Physical Therapy Assistant license.
- Must obtain and maintain CPR certification.
