Therapist - Outpatient Services

Responsibilities

• Provide comprehensive bio-psychosocial assessment services to adults.
• Develop individualized treatment plans with the input of the client, their team, Providers, and natural supports.
• Collaborate with the client’s team to ensure quality of treatment and coordination of care.
• Assess client’s personal, medical, emotional, social, and environmental situation to plan for linkage and treatment course.
• Serve as an advocate for clients, linking them to needed community services.
• Provide individual psychotherapy and skills training to clients from all levels of care.
• Provide case management for assigned clients as needed.
• Develop and facilitate weekly psychotherapy groups.
• Meet with graduate student co-facilitators to ensure effective participation in psychotherapy groups.
• Actively participate in consultation opportunities to ensure effective treatment.
• Maintain documentation and chart in compliance with Medicaid, Commission on Accreditation of Rehabilitation Services (CARF), and agency standards.
• Meet or exceed direct service target.
• Perform other related duties and/or projects as assigned.

Qualifications

• Master’s Degree in Social Work, Psychology, Counseling, or other closely related field required.
• Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, or Licensed Practitioner of the Healing Arts in Illinois preferred.
• Minimum two years’ experience working with people with mental health diagnoses and/or substance use diagnoses.
• Clinical experience providing individual and/or group therapy to adults in a community mental health setting preferred.
