Specific Responsibilities
- Evaluate patient' mental and physical health, substance use, or needs and assess their readiness for treatment
- Work with patients and the multidisciplinary team to develop treatment goals, documented in a treatment plan
- Utilize Measurement-Based Care (MBC) screeners to assess patients, integrates results into treatment planning, and leverages data to track progress and influence care decisions
- Review and recommend treatment options with patients and their families that support full continuum of care
- Help patients develop skills and behaviors necessary to recover from their SUD and/or mental illness
- Work with patients to identify behaviors or situations that interfere with their recovery
- Educate families about SUD and mental illness and help them develop strategies to cope with those problems
- Make recommendations for discharge and aftercare planning and work with Case Management team to ensure safe and adequate care following discharge
- Complete all required documentation in a timely manner
- Perform any other job-related duties as assigned
Education And Experience
- Master's degree or above in Social Work, Mental Health Counseling, or related field required
- Licensure as a (e.g., MHC, LAC, LMFT, LPC, LMHC, LSW, LMSW, LCSW, Psychologist, or equivalent) preferred
- In Florida, if not licensed, CAC or Registered Interns (RMHCI, RCSWI, RMFTI) required. CAP or MCAP preferred
- In Maryland, Masters's-level licensed clinician preferred. If not Masters-level licensed clinician, Alcohol and Drug Trainee (ADT) status required
- One to three years of experience in behavioral healthcare or substance abuse settings serving culturally diverse populations preferred

- Excellent verbal and written communication skills required
- Excellent organizational skills with the ability to prioritize workload and meet deadlines
- Must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands
- Exceptional analytical and problem-solving skills
- Proven ability to handle multiple projects under pressure
- Strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint)
- Exhibits excellent customer relations skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families, and referral sources
