Responsibilities
- Accountable for overall treatment planning efforts, care conferences with families and providers, family and provider care updates, psychosocial needs and completion.
- Carefully coordinate and facilitate the development of a discharge plan of care for the patient population which includes accountability for the complex discharge process.
- Ensure the experience of care for the patient and family is exceptional.
Qualifications
- Education: Master's in Social Work (MSW), Counseling, Psychology, or related human services field is required.
- Candidates with a Master’s Degree in Counseling: Associate license OR full counseling license required. Candidates with a Master’s Degree in Social Work: associate or temporary license in the state of employment required, full license preferred.
- Minimum of 2 years of clinical experience in a behavioral healthcare or acute psychiatric setting preferred.
- Previous experience in an acute in-patient behavioral health hospital is a plus.
- Experience developing and administering treatment plans.
- Individual and group counseling techniques.
- Exceptional communication skills.
