Responsibilities

  • Collaborates with the treatment team to implement goals and objectives in each youth’s treatment plan.
  • Advocates for and coordinates resources that support emotional and mental health needs.
  • Collaborates with community and state agencies (e.g., DMH, DCF, DYS, DPH, MCI) to facilitate service access and care coordination.
  • Participates in Care Planning Team meetings when youth are enrolled in ICC; aligns services with wraparound principles.
  • Maintains timely, accurate documentation and completes all required data entry per payer and agency standards.
  • Provides culturally responsive, trauma-informed services that respect each family’s strengths and needs.
  • Participates in supervision, team meetings, and quality/process improvement activities.

Requirements

  • Associate’s and/or Bachelor’s degree in a human services field with 1–2 years of experience working with children, adolescents, and transition-age youth.
  • Experience developing and facilitating training programs for families and/or staff.
  • Valid driver’s license and reliable car are required.
  • Ability to follow written and verbal directions effectively; strong communication and documentation skills.
  • Commitment to equity, cultural responsiveness, and family-centered care.

Pay

$26.45 per hour.

Schedule

Full-time role (40 hours/week) with availability during after work and after school hours between 2 – 8 pm Monday through Friday.
