For patients in all other states, this is known as “Charlie Health Medical, P.A.”

Responsibilities

• Support clinical outreach efforts to connect patients with appropriate care
• Collaborate with clinical teams to ensure seamless patient care coordination
• Manage patient referrals and maintain accurate records
• Participate in quality assurance and improvement initiatives

Requirements

• Bachelor's degree in psychology, social work, healthcare administration, or related field
• Experience in healthcare or mental health field preferred
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple tasks

Qualifications

• Valid driver’s license and reliable transportation
• Proficiency in Microsoft Office Suite
• Knowledge of HIPAA and confidentiality requirements

Pay

Salary range: $50,000 - $80,000 annually

Full-time position, Monday through Friday, 9:00 AM - 5:00 PM
