Temporary Social Services Designee

About the Role

Coordinates the overall interdisciplinary plan of care for residents, from admission to discharge. Acts as a liaison between resident/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Coordinates the overall psychosocial care plan for residents. Provides support and advocates for resident/patient and their family. Develops and maintains community resources for residents.

Qualifications

• Related coursework in social work or psychology
• Hospital or nursing home experience, preferred
• Able to read, write and communicate in English

• Pay: $25.00 - $27.00
