TEMPORARY Recovery Case Manager (12 week assignment)

The Recovery Case Manager (RCM) will meet with clients regularly on a one-on-one basis, serve as the primary contact for a client's family, loved ones, and referral sources, and be responsible for planning and implementing the client’s aftercare.
Responsibilities
- Help hold clients accountable for their actions in the community and the completion of their work
- Facilitate and provide timely documentation
- Monitor compliance on client contracts, phone policy, and “buddy” relations
- Ensure that all pertinent information is notated on RSS office board at shift change
- Work in a fast-paced, dynamic environment
- Communicate effectively in written, electronic, and oral form
- Make sound decisions and/or find alternative solutions
- Make weekly calls to the client’s family (more communication may be required), logging all communication in the EMR. Calls will address client progress, behavior, contracts, growth opportunities, programming, and aftercare. Be prepared to discuss enabling, codependency, and/or Al-Anon
- Schedule all calls and face-to-face meetings with family members and/or loved ones
- Conduct family calls within the first 24 hours of admission
- Make referral calls within the first 24 hours of admission and weekly thereafter
- Meet with clients at least once a week to monitor the treatment process
- Execute step downs and discharges
- Upload/delegate current ROIs and aftercare plans into EMR
- Attend Morning Meeting, RCM Meeting, Supervision (when necessary), Safety Squad, and Quality Assurance meetings
- Engage with clients beyond individual caseload
- Introduce yourself to new clients on caseload within 48 hours of admission; log interaction in the EMR
Requirements
- High school diploma or GED required
- Extensive experience within Mental Health and Substance Use Disorder industry
- Time management skills and the ability to work on multiple projects simultaneously
- Coordinate with all departments and oversee clients' stay at GMTC
- Create an after-care plan that realistically meets the needs of the client
- Vet client visitors and phone calls
- Meet and maintain all pre-employment requirements, including reference checks, applicable state and national background screening, 10-panel urine drug screen, DMV check, insurance (if applicable), and license/credential verifications
