About the Role
Telephone Triage LVN will be responsible for patient assessments and addressing patient needs by responding to patients over the phone line. Collaborates with providers to ensure quality patient care in compliance with local, state, and federal regulations and accreditation standards.
Responsibilities
- Monitors, prioritizes, and responds to triage needs on the phone line.
- Responds to patient inquiries, prioritizing responses and routing urgent situations to a physician or provider.
- Talks directly to patients on the telephone to schedule appointments with their physician during office hours.
- Maintains accurate and detailed documentation and follows patient privacy in accordance with HIPAA and company policies and procedures.
- Arranges appointments for patients who need to see a physician and consults with physicians as needed.
- Delivers patient education according to the individualized needs of the patient and as prescribed by the provider and/or group policy.
- Serves as a clinical resource and professional role model for other staff.
- Prioritizes work tasks and services and completes tasks in a timely manner.
- Maintains accurate, clear, and concise progress notes, problem lists, and medication lists.
- Communicates respectfully with staff, consultants, and patients.
- Performs other duties as assigned.
Requirements
- Associate’s degree in nursing.
- Active and valid LVN license in good standing from the state the practice is located in.
- One (1) year triage experience preferred.

Equipment Operated
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Work Environment & Physical Requirements
