Primary Responsibilities

  • Collaborate with providers to identify needs for preventive care, evidence-based chronic disease management and care coordination.
  • Provide general and specific healthcare education to patients.
  • Identify and monitor self-management goals for patients.
  • Collaborate care with community based providers.
  • Follow up on ED visits and inpatient discharges.
  • Assist providers with exams, treatments and procedures as needed.
  • Perform telephonic triage and assessment, education, and coordination of healthcare and community resources.
  • Directly provide patient care, including Annual Wellness Visits (AWVs) and Chronic Care Management (CCM).
  • Manage INR monitoring and teaching for patients on Coumadin.
  • Administer vaccines and medications, providing patient education as appropriate.
  • Perform a wide range of nursing activities under the direction of the provider.
  • Maintain and ensure follow-up of all abnormal test and imaging results.
  • Oversee inventory and ordering management of clinical supplies and equipment.
  • Delegation and oversight of unlicensed staff, including MAs, CNAs, and nursing students.
  • Ensure compliance with organizational mandatory annual training.

Required Qualifications

  • Graduate from an accredited Nursing Program required. Bachelor's degree preferred.
  • Licensure as a Registered Nurse in Massachusetts required and Certificate 1 Basic Life Support required.
  • One to three years related work experience required. New graduate RN’s considered.
  • CPR Certification.
  • Strong interpersonal and patient communication skills.
  • Advanced skills with Microsoft applications.

Physical Requirements

  • Medium work: Exerting up to 50 pounds of force occasionally and up to 20 pounds of force frequently.
  • Constant sitting, walking, endurance-working up to 3-4 hours without a break, frequent standing, bending neck, bending waist, power grasping using one hand, fine manipulation using both hands, pushing/pulling using both hands, keyboard use, reaching-above shoulder height, reaching-below shoulder height.
  • Rarely pull up and/or reposition patient weighing up to 100 pounds without assistance, pull up and/or reposition patient weighing up to 250 pounds with assistance.

Pay Range

$32.24 – $55.32
