Team Nurse - Hybrid Remote

About the role

This is a hybrid remote position with occasional float in-person at our Mount Auburn Primary Care offices in the Cambridge area.

Principal Duties and Responsibilities

• Act in compliance with nursing philosophy, practice standards, and qualifications.
• Collaborate with physicians and midlevel providers in providing direct healthcare to patients.
• Evaluate client outcomes and condition changes.
• Communicate and document teaching and follow-up care with patients.
• Document patients' understanding of plan/instructions.
• Communicate pertinent patient information and findings to providers, associate nursing peers, and other caregivers in a timely manner.
• Triage telephone inquiries, review lab results and diagnosis testing, and assure written patient notification of test results, missed appointments, and abnormal study follow-up when appropriate.
• Arrange referrals to ancillary services.
• Demonstrate competency in office including testing, phlebotomy, and assist physician/midlevel provider with procedures.
• Maintain and supervise the integrity of departmental logs.
• Review and comprehend Department of Public Health and Welfare information and recommendations for prophylaxis, treatment, and reporting of communicable diseases.
• Complete informational forms such as premarital and WIC.
• Recognize signs and symptoms of child and domestic abuse and report accurately.
• Maintain stock of client education materials and review new material available.

Organizational Responsibilities

• Follow hospital and departmental policies with special attention to safety, dress, attendance and punctuality, and display of ID badge.
• Subscribe to hospital's mission statement, code of ethics, and standard of conduct.
• Handle all work-related information in a confidential manner, as outlined in Confidentiality Policy B.91.
• Recognize and respect each patient's rights to privacy, and understand that this is protected by statute.
• Understand that looking up patient information not needed for job duties is prohibited and is grounds for immediate dismissal.
• Treat customers with courtesy and compassion and respond to requests in a timely manner.
• Work effectively with members of the team so that they meet constantly changing needs and deliver care that is customer oriented and cost effective.
• Demonstrate initiative and creativity to continuously improve services, departmental and interdepartmental processes, and any other activities that affect quality.
• Plan, organize, and render care for neonate, pediatric, adolescent, and geriatric patients within an assigned area.
• Perform work responsibilities while staying in compliance with all applicable laws, rules, and regulations.
• Understand and agree to uphold the Hospital Compliance Plan.

Patient Safety

• When taking a telephone order, write the order on the Doctor's Orders, read the order back to the MD, and document as a TORB.
• When taking a verbal order, repeat the order back to the MD prior to implementing the order and document as a VORB.
• Use only accepted abbreviations when documenting in the medical record.
• Use two unique patient identifiers prior to treatment or patient transport.
• Consistently use sharps devices as designed to prevent injury.
• Comply with Isolation Guidelines; wear appropriate personal protective equipment as required by specific patient precautions.
• Consistently comply with CDC Handwashing Guidelines.
• Clean hands before and after patient contact, or contact with patient environment.

Qualifications

• Knowledge of nursing process, theories, and clinical expertise related to patient care, normally acquired through the completion of a nursing training program at an NLN accredited school of nursing.
• Current registration in the Commonwealth of Massachusetts.
• Interpersonal skills to interact with patients, families, and a multidisciplinary team of healthcare personnel to resolve complex problems.
• Able to acquire, through training programs and continuing education, technical knowledge related to the area.

Working Conditions

• Standard office environment.
• Exposure to infectious diseases, chemicals, and other toxic agents.
• Computer skills required.

Pay

$32.24 - $55.32 per hour. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
