Team Leader, Clinical Services, Homecare (Full-Time, Days)

Responsibilities

• Leads orientation and training for new staff
• Provides clinical guidance and supports ongoing development
• Maintains individual performance monitoring
• Ensures timely and accurate referrals
• Guides case management and oversees service delivery
• Maintains documentation standards
• Participates in developing in-service programs and staff education sessions
• Adheres to agency standards, privacy regulations, and corporate ethics
• Represents the agency in community meetings
• Conducts staff meetings
• Ensures attendance reliability
• Performs additional functions as assigned by management

Requirements

• Bachelor's degree aligned with license
• One year of experience working in a professional Home Care setting
• Valid NYS license in Social Work, Physical Therapy, Occupational Therapy, or Speech Therapy
• Knowledge of current community health nursing practices
• Strong leadership/supervisory skills
• Solid interpersonal, organizational, and time management skills
• Ability to learn and utilize new systems, computer applications, and operating environments as necessary
• Must have ability to travel
