Clinical Coordinator, Registered Nurse, Home Care Team 4, Full Time, Day

About the role

To oversee and coordinate the overall clinical operations, functions, and activities of the Home Care Team, including ensuring efficient processing of referrals, evaluating services, handling scheduling (including weekend scheduling), and supervising assigned staff.

Education

Graduate from a NLN accredited School of Nursing. BSN or Associate RN from a NLN accredited school preferred.

Experience

At least two (2) years experience as an RN in Home Care or community health. Supervisory experience preferred.

Skills

• Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing)
• CPR (BCLS) Certification required
• Effective interaction and communication (oral, writing, presenting) skills
• Effective organization skills and ability to perform work accurately with attention to detail, managing multiple tasks without loss of efficiency or composure
• Ability to function competently in stressful situations and adapt to changing patient needs, including acute, chronic, and complex disease processes and end‑of‑life care
• Ability to work cooperatively within the health system, with patients and families, and with multidisciplinary team members (physicians, rehab, lab, etc.)
• Effective time management to set priorities, perform job‑related responsibilities, and respond quickly to emergency situations
• Critical thinking ability
• Current and valid driver’s license, registration, inspection sticker, and appropriate insurance coverage (including collision)
• Adherence to HR Policy 108, maintaining an acceptable driving record while driving is a job responsibility
