Team Leader

About the role

Supervise and provide multi-disciplinary clinical and case management services delivered by a team of clinical and administrative staff, with a focus on assertively engaging hard-to-reach consumers with serious mental illness. Ensure that the needs of consumers, ACT program goals, and contract objectives are effectively achieved. This role involves considerable professional initiative and judgment.

Responsibilities

• Manage day-to-day clinical operations and required productivity standards.
• Direct and coordinate the consumer admission process, treatment and rehabilitation services, support services, and discharge process.
• Facilitate team and treatment planning meetings.
• Establish and implement procedures for monitoring workflow and promoting enhanced staff performance.
• Compile the weekly report, Service Plan e-tracker, and weekly census reports to SPOA.
• Assist with conducting other required assessments with clients, such as Child and Adult Integrated Reporting System (CAIRS) reports, Assisted Outpatient Treatment (AOT) reports, and Home and Community Based Services (HCBS) eligibility assessments.
• Manage communication with MCOs to authorize services.
• Provide direct supervision of team members; handle complaints and grievances.
• Monitor service delivery and the impact and effectiveness of services, as well as consumer satisfaction.
• Schedule staff work hours to ensure appropriate coverage; monitor and approve timesheets.
• Assist with scheduling 24-hour crisis intervention on-call services on a rotating basis.
• Participate in staff recruitment, interviewing, hiring, orientation, and performance plans.
• Ensure the use of evidence-based treatments in the areas of mental health and substance abuse to address client needs (e.g., cognitive behavioral therapy, harm reduction, motivational interviewing, trauma-informed care).
• Set standards of performance by overseeing all documentation to ensure quality, timeliness, and accuracy, in compliance with Medicaid, NYS Office of Mental Health, NYC Department of Health and Mental Hygiene, and agency standards.
• Adhere to ACT policies and procedures, agency policies, and maintain appropriate professional standards.
• Participate in the quality improvement process, administrative meetings, team meetings, and in-service trainings.
• Perform other duties as required by the Director of ACT Services.

Skills and knowledge

• Demonstrated skill in managing difficult situations and prioritizing competing demands.
• Delivering services in community-based settings and comfort with home visits.
• Excellent written, oral, presentation, and facilitation skills.
• Ability to effectively collaborate with staff across the agency and to shift roles from Team Leader to direct service delivery.
• Ability to function in a fast-paced environment.

Qualifications

• Master's Degree required: LMHC, LMSW, LCSW, or equivalent licensure in New York State.
• At least two years of experience working in human services with individuals with serious mental illness, and with those with serious mental illness and co-occurring substance use disorders.
• Must be outcome-oriented in a fast-paced environment with deadlines.
• Able to work in the Microsoft Office Suite; proficient in Word, Outlook, and Excel.
• Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
