About the role
The PICL Program at Resources for Human Development (RHD) in the Montgomery County area of Pennsylvania provides community-based behavioral health services. This role involves leading a team of direct care staff, providing supervision, and ensuring the successful implementation of support plans for individuals served.
Responsibilities
- Provide first-line supervision, coaching, mentoring, and support to direct care staff.
- Organize, schedule, and distribute work assignments among team members.
- Maintain a direct service caseload and provide direct support services as needed.
- Cook up and oversee medical appointments, including attending appointments when necessary.
- Communicate with medical providers regarding the health and wellness needs of individuals served.
- Maintain positive relationships with family members, guardians, staff, and program leadership.
- Participate in person-centered planning meetings and ensure implementation of support plans.
- Coordinate training plans and ensure accurate and timely documentation.
- Monitor progress notes, daily documentation, and monthly reviews.
- Communicate regularly with service coordinators and interdisciplinary team members.
- Prepare and submit reports and required program documentation.
- Support continuous quality improvement initiatives.
Qualifications
- Current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification.
- Valid Driver's License.
- One of the following:
- Bachelor's Degree in a Human Services field such as sociology, special education, rehabilitation, nursing, counseling, psychology, or a related field.
- Associate Degree and three (3) years of experience supporting individuals with intellectual disabilities.
- High School Diploma or GED and five (5) years of experience supporting individuals with intellectual disabilities.
Preferred Qualifications
- Previous supervisory or team leadership experience.
- Experience working within behavioral health, psychiatric rehabilitation, or community support programs.
Physical Requirements
- Ability to lift up to 50 pounds.
- Ability to stand, sit, walk, bend, reach, and climb stairs as needed.
- Ability to push, pull, kneel, stoop, crouch, and perform repetitive movements.
- Ability to perform essential job functions with or without reasonable accommodation.
Schedule
- Full-Time.
- Flexible schedule based on program needs.
- Evening and weekend availability may occasionally be required.
- Local travel may be required to support individuals and attend appointments.
Pay
$21.12 per hour.
