Teacher - Special Education - Transition Facilitator/Mild Disability (MID) - Mountain View High School

Key Responsibilities

• Serve as interagency liaison between the youth, the family, the school, community and state agencies, and employers
• Coordinate, administer and/or interpret career development and assessment tools, working with the youth, family, special educator, career and technical educator, and counselor when appropriate
• Coordinate program development at the junior and high school levels which support transition planning and provide a continuum of career development and life planning activities
• Function as a resource for classroom teachers in providing mini-units and/or co-teaching opportunities of employability skills, self-advocacy skills, and independent living skills
• Provide coordination and reporting of vocational placement and follow-up for Mesa Public Schools
• Facilitate IEP meetings at the junior and high schools related to transition service needs of students
• Assist youth in overcoming barriers to job search activities and in obtaining employment
• Inform parents and families of community resources to facilitate access and adult agency service coordination

Required Qualifications

• Bachelor’s degree in vocational evaluation, counseling, special education, psychology, transition, or related field
• Master’s degree (Preferred) in vocational evaluation, counseling, special education, psychology, transition, or related field
• Valid Arizona Special Education teaching certificate
• Five years experience in special education, vocational training, or related area providing services to youth with disabilities
• Experience in aligning special education and general education systems (curriculum, assessment, etc.) for student-driven transition outcomes

• Ability to meet deadlines, work on multiple projects, coordinate work with others, and be self-directed
• Knowledge of components of teaching self-advocacy and self-determination skills to any youth with a disability
• Ability to develop and maintain cooperative and effective professional relationships
• Communicate effectively (verbal and written) with diverse community members (e.g. youth, families, educators, adult service providers, employers)

Work Environment & Physical Requirements

• Office and classroom setting within the district or on a school campus
• May travel to schools, homes, job sites, or community centers to meet clients and coordinate resources
• Frequent interactions with clients, families, educators, employers, and service providers
• May require evening or weekend meetings to accommodate client availability
• May require walking, standing, or traveling between different locations
• Prolonged sitting for administrative tasks, documentation, and meetings
• Occasionally you may need to transport documents, equipment, or assistive devices up to 20 lbs.
