Targeted Case Manager, TCM

Responsibilities

• Provides case management to Health Home eligible clients, adults with severe mental illness, and adults with recent SUD admissions.
• Offers individual counseling and facilitates caseload group meetings and other assigned groups.
• Supports clients in accessing community services and social support.
• Assists clients in navigating the health care and social services systems.
• Supports clients and advocates for necessary services.
• Engages in on-site crisis intervention and skills teaching.
• Maintains oversight of the treatment plan to ensure it addresses individual needs and wellness goals.
• Carries out monitoring and follow-up activities through contacts with service recipients, families, and other relevant parties.

Qualifications

• Bachelor’s Degree in Human Services required.
• Masters in Social Work preferred.
• Bilingual Spanish proficiency preferred.
• Minimum 2+ years of experience in counseling clients with severe mental illness and/or substance use disorders.
