About the role

Targeted Case Manager providing case management services to individuals who meet the definition of being developmentally disabled, as defined by the State of Montana. Pay range: $22.00 - $27.33 per hour to start.

Responsibilities

• Assist individuals in gaining access to:
  • Medical Services
  • Social Services
  • Educational Services
  • Vocational Services
  • Recreational Services
• Manage caseload and/or billing requirements on a monthly, quarterly, and yearly basis.

Requirements

• BS/BA Degree in a social work related field.
• Minimum of one (1) year of direct work experience in the developmental disability field.
• Ability to develop and implement individual treatment plans with knowledge of behavioral, clinical, and mental health interventions, and skill-building techniques.
• Understanding of the emotional and behavioral needs of assigned clients.
• Strong interpersonal communication skills necessary to:
  • Work independently and as a team member.
  • Make observations and report them accurately.
  • Follow oral and written instructions.
• Ability to facilitate meetings.
• Ability to work independently via a virtual office using a laptop computer and cell phone.
