About the role
Join Starkey as a Targeted Case Manager and become an advocate, resource connector, and support partner for individuals with intellectual and developmental disabilities.
Responsibilities
- Coordinate and monitor services for individuals on your caseload
- Advocate for individuals and help them access resources and supports
- Build strong relationships with families, guardians, providers, and community partners
- Aid in person-centered planning and goal achievement
- Serve as the primary point of contact and referral source for assigned individuals
- Ensure services are meeting needs and supporting independence, community integration, and quality of life
Requirements
- Bachelor's degree in Human Services or a related field
- OR five (5) years of related, verifiable full-time experience in developmental disability services in lieu of a degree
- Minimum six (6) months of experience in developmental disability services
- Valid driver's license with a good driving record
- Ability to successfully complete the Targeted Case Management state test within the required timeframe
Preferred
- One (1) year of case management experience
