Tailored Care Manager
About the role
At Easterseals PORT Health, we're dedicated to improving the lives, health, and well-being of individuals and families impacted by substance use and mental health challenges. As a Tailored Care Manager, you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Clinton, NC and surrounding counties, helping close gaps in care and improving lives in the community every day.
Responsibilities
- Evaluate client needs and develop personalized care plans
- Coordinate services addressing physical and mental health, social determinants of health, and other life challenges
- Provide services in various community settings
- Participate in rotating after-hours support
Requirements
- Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area (Master’s level degree preferred)
- Eligible for Qualified Mental Health Professional (QMHP) credentialing
- Minimum 2 years of experience with mental health and/or substance use populations
- Proficiency in Microsoft Office and Electronic Health Records (EHR)
- Valid driver’s license, current auto insurance, and a clean driving record
- Strong communication skills and the ability to work independently

Pay
Competitive salary ranging from $53,000 to $56,000 annually, based on experience and qualifications.
Schedule
Full-time, primarily Monday–Friday from 9 AM to 5 PM, with rotating on-call responsibilities.
