Tailored Care Manager

We're currently looking for a compassionate and committed Tailored Care Manager to become part of our supportive, multidisciplinary Tailored Care Management (TCM) team in Wilmington, NC. About the role
As a Tailored Care Manager, you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This hands-on role involves traveling throughout Wilmington, NC and surrounding counties to help close gaps in care and improve lives in the community every day.
Responsibilities
- Evaluate client needs and develop personalized care plans.
- Coordinate services addressing physical and mental health, social determinants of health, and life challenges.
- Provide services in various community settings.
- Participate in rotating after-hours support.
Requirements
- Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services field (Master’s level degree preferred).
- Eligible for Qualified Mental Health Professional (QMHP) credentialing.
- Minimum 2 years of experience with mental health and/or substance use populations.
- Proficiency in Microsoft Office and Electronic Health Records (EHR).
- Valid driver’s license, current auto insurance, and a clean driving record.
- Strong communication skills and the ability to work independently.

Pay
$54,000 annually.
Schedule
Full-time, non-traditional Monday–Friday schedule with rotating on-call responsibilities.
