Responsibilities
- Building trusting relationships with clients and providing direct support in areas like housing, finances, vocational, educational, legal, and medical needs.
- Coordinating case management by making referrals and collaborating with community agencies and professionals.
- Completing documentation, communicating with partners, and offering crisis intervention and ongoing client advocacy.
- Developing community connections with officials, families, employers, and organizations to ensure comprehensive support.
Qualifications
- Two years of experience working with individuals with mental illness or a Bachelor's/Master’s in Human Services, Psychology, Social Work, or related fields.
- Social Work licensure for Social Work candidates.
- At least one year of professional or internship experience in mental health.
- Valid Minnesota Driver’s License and access to reliable transportation.
