Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Affirm patients' wishes regarding finances, funeral arrangements, and other end-of-life matters.
• Navigate the grieving process for patients' loved ones.
• Offer counseling and crisis intervention services.
• Provide bereavement support and grief counseling.
• Educate patients and families on the emotional and psychological response to illness progression.

Requirements

• Master's of Social Work from a Council on Social Work Education-accredited school, or equivalent experience.
• Reliable transportation.
• At least one year of social work experience in a healthcare setting.
• Experience working with terminally ill patients and their families.
• Excellent listening and communication skills.
• Ability to be an effective patient and family advocate.
• License and/or certification required by state minimum standards.

Qualifications

• Licensure as a Licensed Social Worker (LSW) in applicable states.
• Knowledge of key psychosocial issues related to terminal illness and their impact on patients and families.
• Capacity to work effectively within an interdisciplinary team.
• Empathy and compassion towards seriously ill patients and their families.
