Requirements

• Passing a background and urinalysis test
• CPR certification
• Valid auto insurance
• Up to date negative TB test or chest x-ray
• Valid driver’s license
• Reliable transportation
• Ability to adapt to a variety of environments

Qualifications

• HHA’s and PCP’s able to meet the following qualifications:
