About the role
The role involves conducting comprehensive participant assessments and reassessments, working with social workers to develop and monitor care plans for the elderly, and facilitating the delivery of necessary health-related services.
Responsibilities
- Conducts comprehensive participant assessments to determine health care needs.
- Works with the team social worker to develop participant care plans, including determining the frequency and duration of required services.
- Facilitates the delivery of services under the care plan with various healthcare providers and organizations.
- Maintains and monitors participant care plans to ensure the delivery of necessary services and conducts reassessments as needed.
- Works with family members and caregivers to enhance available informal support systems.
- Maintains relationships with hospitals, human service providers, home care agencies, and other relevant agencies.
- Ensures compliance with all program standards and guidelines.
- Provides documentation of project activities for reports and participant applications to funding sources.
- Handles other duties as assigned.
Requirements
- Excellent interpersonal and group process skills.
- Accurate, conscientious, and flexible with attention to detail.
- Familiar with computers.
- Accepts direction, is well organized, and meets deadlines.
- Good handwriting.
- Willingness to work in a team environment.
- Valid driver’s license and reliable transportation.
- Experience in a non-profit environment with a community health and/or human services organization working with older adults.
- Familiarity with the aging network, community organizations, and resources for seniors.
- Some experience with data entry and data management.
- Familiarity with third-party reimbursement systems.
Education
- Registered Nurse with current state of Michigan License.
- Bachelor's of Science degree in nursing from an accredited four-year college or university is preferred.
