Scope
Responsible for coordinating the intake process of new patients for supportive care services such as nutrition assistance, medical equipment/supplies and transportation assistance.

Qualifications
- High school diploma required
- Some college preferred
- Minimum five (5) years' social services experience
Specialized knowledge
- Experience in inventory control
