Responsibilities

• Provide individualized support to individuals experiencing homelessness
• Coordinate services such as housing, healthcare, and employment assistance
• Assess client needs and develop action plans
• Ensure clients are connected to appropriate community resources
• Maintain accurate records and report progress to supervisors

Requirements

• Bachelor’s degree in social work, psychology, or related field
• At least 2 years of relevant experience in homelessness services or related field
• Valid driver’s license and access to reliable transportation
• Ability to work flexible hours including evenings and weekends

Qualifications

• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite
• Knowledge of local resources and service providers
