Job Summary

A Supportive Home Care Worker is needed in Crandon, WI. This role requires training in providing personal care services, including assistance with various daily activities.

Job Requirements

• Personal Care Worker (PCW) responsibilities include assisting with bathing, toileting, dressing, mobility, transfers, and changing bed linens and personal clothing.
• The PCW must report changes in the consumer's condition promptly to the Registered Nurse (RN) Case Manager.
• Accurate and timely documentation of all time sheets and other necessary paperwork is required.
• Maintaining confidentiality of consumer information is a key responsibility.
• The position involves extensive hands-on personal care and requires physical strength, including the ability to lift and transfer individuals using transferring techniques or assistive technology.
• Physical requirements include regular sitting, standing, walking, handling objects, reaching, bending, squatting, twisting, talking, and hearing.
• Good communication and interpersonal skills are essential.
• Reliable transportation to the worksite is required.
• Completion of an interview and training conducted by PAS staff is mandatory.
• Satisfactory criminal background results are necessary.

• Ability to understand and follow consumer instructions.
• Good communication skills.
• Ability to relate well to others.
• Good past employment references.
• Physical strength sufficient to perform essential job functions.
