About the role

Trillium Health Resources is seeking an experienced Support Transition and Reentry (STAR) Manager to join the Support Transition and Reentry (STAR) Team. STAR is a specialty care team serving incarcerated adults with complex behavioral health conditions. This leadership role provides direct supervision and oversight to ensure the delivery of high-quality, person-centered integrated care. The ideal candidate has experience leading a team, care management expertise, and is familiar with criminal justice systems, reentry programs, and community-based services.

Office location: Remote within 75 miles of Trillium's Catchment of North Carolina.

Responsibilities

• Provide direct supervision and support to QP level coordinators, licensed clinicians, and certified peer support specialists.
• Monitor team's quality of work and take corrective action as needed.
• Oversee complex cases ensuring sound decision making and wholistic client care.
• Support staff to effectively plan and coordinate member transitions from incarceration to the community.
• Collect data, assign cases, and conduct quality audits.
• Collaborate with internal departments and external systems including prisons/jails, probation/parole, BH/SU providers, and other stakeholders.
