Responsibilities

• Provide person-centered care to individuals with diverse abilities in community and home settings.
• Assist with daily living activities, personal care, medication reminders, and transportation.
• Support community integration and promote independence, dignity, and safety.
• Implement care plans and document services accurately.
• Support behavioral and emotional needs of individuals.
• Deliver personalized care, promote safety and dignity, support community engagement, and ensure documentation compliance in a supportive and inclusive environment.
