Support Professional - Developmental Disabilities

Responsibilities

• Provide person-centered care to individuals with diverse abilities in home and community settings.
• Assist with daily living activities, health and safety, community participation, transportation, and skill-building to promote independence and improve quality of life.
• Work closely with an interdisciplinary team to follow individualized support plans.
• Document services and communicate changes in needs or behavior effectively.
