Support Coach - TRAILS

About the Role

The Support Coach is a direct care level person responsible for delivering a full range of support services to individuals impacted by behavioral health issues (addiction, substance use, and mental health). The role involves recognizing that clients often have experienced trauma, and providing services in a trauma-informed, sensitive, and responsive environment to promote improved wellbeing. The Support Coach facilitates the maximum level of independent functioning by teaching and role modeling skills, demonstrating a warm, empathic, and hopeful attitude, and conveying a philosophy of dual recovery.

The Support Coach reports to the Residential Coordinator (or designee).

Responsibilities

• Ensure every client is where they should be during the shift, encourage participation in scheduled activities, and accurately record client actions and observations in daily progress notes.
• Prepare and serve appealing meals and snacks according to established menu plans and timelines.
• Accurately prepare, distribute, and document medications per each client’s medication administration record (MAR) and physician’s orders.
• Help, instruct, and guide clients in carrying out daily tasks in accordance with their Intensive Residential Treatment Services (IRTS) program and treatment plan.
• Teach and model appropriate conduct and coping mechanisms to clients in the context of building relationships.
• Plan and engage clients in social skills activities to encourage purposeful activity and reduce isolation.
• Complete routine cleaning, housekeeping, and other assigned duties as directed by the TRAILS Program Supervisor or designee.
• Accurately finish all assigned paperwork before the end of the shift and communicate any client issues to the supervisor during shift changes. Ensure all documentation is completed within required time frames and complies with program, CARF, state, federal, and quality standards.
• Immediately contact the TRAILS Program Supervisor (or designee) in the event of:
  • Any client or staff injury or need for ER/hospitalization.
  • Death of a client.
  • Suspected abuse or neglect of a client.
  • Any other situation requiring notification as dictated by policy and procedure.
• Perform all other duties as assigned.

Requirements

• High school diploma or equivalent required.

Preferred Qualifications

• A minimum of six (6) months full-time employment experience as an agency homemaker, certified nurse aide, or a minimum of one (1) year experience caring for children, elderly, or physically/mentally ill individuals.
• Successful completion of formal training in nursing arts, or as a Certified Nurse Aide or Home Health Aide, can substitute for the qualifying experience.

Physical Requirements

• Work in a normal direct care environment, which may require day, evening, overnight, and/or weekend shifts.
• Frequent standing, bending, stooping, and occasional lifting of up to 50 pounds.
• Normal range of hearing and manual dexterity sufficient to assist clients in day-to-day living routines.
• Ability to work under stressful conditions and irregular hours.
• Use of a computer.
• Drug screen and TB test upon hire, with annual TB tests thereafter.
