Support Coach - PRN

About the Role

The Support Coach is a direct care level person responsible for delivering a full range of support services to individuals impacted by behavioral health issues (addiction/substance use and mental health). The role recognizes that clients often have experienced trauma affecting their development and adjustment. The Support Coach facilitates the maximum level of independent functioning by teaching and role modeling skills to promote independence, demonstrating a warm, empathic, and hopeful attitude while conveying a philosophy of dual recovery. Services are provided in a trauma-informed care environment to promote improved wellbeing.

The Support Coach reports to the Adult Residential Administrator.

Responsibilities

• Ensure every client is where they should be during the shift, encourage participation in scheduled activities, and accurately record client actions and observations in daily progress notes.
• Prepare and serve appealing meals and snacks according to established menu plans and timelines.
• Accurately prepare, distribute, and document medications per each client’s medication administration record (MAR) and physician’s orders.
• Help, instruct, and guide clients in carrying out daily tasks in accordance with their Intensive Residential Treatment Services (IRTS) program and treatment plan.
• Teach and model practical coping mechanisms and conduct control to build client relationships.
• Plan and engage clients in social skills activities to encourage purposeful activity and reduce isolation.
• Complete routine cleaning, housekeeping, and other assigned duties as directed.
• Finish all required paperwork before the end of the shift and communicate client issues to the supervisor during shift changes. Ensure documentation is completed within allotted time frames and complies with program requirements, CARF, state, federal regulations, and quality standards.
• Immediately contact the Adult Residential Administrator (or designee) in the event of:
  • Client or staff injury requiring ER/hospitalization.
  • Death of a client.
  • Suspected abuse or neglect of a client.
  • Any other situation requiring notification as dictated by policy.
• Perform all other duties as assigned.

Requirements

• High school diploma or equivalent required.
• Knowledge and skills working with individuals with serious and persistent mental illness.
• Ability to communicate effectively with clients and understand their special problems and needs.

Physical Requirements

• Work in a normal direct care environment, including evenings and/or weekends.
• Some travel may be required for appointments and community outings.
• Frequent standing, bending, stooping, and occasional lifting up to 50 pounds.
• Normal range of hearing and manual dexterity sufficient to assist clients in day-to-day living routines.
• Ability to work under stressful conditions and irregular hours.
• Use of a computer.
• TB test required upon hire.
