PRN LPN Health Coach - Staffing Support

Key duties

• Determines all Medicare eligible patients for Annual Wellness Visits and subsequent AWVs using the Practice Dashboard.
• Screens patients to facilitate early interventions on positive findings.
• Accurately bills all services performed at the practice to the biller/coder.
• Provides effective education to patients promoting health and wellness.
• Shares education and information with the practice team relevant to the medical home's goals.
• Performs pre-visits and patient outreach to ensure care gaps are closed.
• Monitors quality dashboards to ensure care gaps are being addressed.
• Affirms care documentation accuracy to promote patient safety.
• Timely addresses care concerns from annual wellness visits with providers and/or office managers.
• Accurately completes charts with same-day service documentation.
• Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs.
• Participates actively in multi-disciplinary team meetings.

Qualifications

• Program Graduate, Licensed Practical Nurse (Required)
• 1 year Nursing experience (Required)
• Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP), required upon hire
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC), required
