Substance Use Recovery Case Manager

Role: Transition Recovery Home Case Manager Responsibilities: Provide person-centered case management for adults in early recovery within a community-based residential setting. Develop and monitor individualized recovery plans, coordinate mental health, addiction, medical, housing, and social services. Offer daily living support and relapse prevention strategies. Collaborate with an interdisciplinary team to ensure comprehensive care. Document services accurately in electronic health records (EHRs) and practice trauma-informed, recovery-oriented care. Qualifications & Skills: Strong interpersonal and communication skills, experience in case management or related fields, knowledge of mental health and addiction services, ability to work collaboratively, and commitment to trauma-informed care in a mission-driven environment.
