Role: Blount Transition Recovery Home Case Manager Responsibilities Conducts intakes and assessments to evaluate client needs.Develops individualized service and recovery plans.Coordinates mental health, addiction treatment, housing, and social services.Monitors client progress and advocates on their behalf.Maintains accurate documentation of services and client outcomes.Supports clients in building stability, life skills, and community connections.
