The Substance Use Navigator (SUN) is a trained, Certified Peer Recovery Specialist who supports patients in outpatient settings to access evidence-based treatment for substance use disorders with a focus on harm reduction and medication-assisted recovery (MAR). They conduct initial assessments, connect patients to recovery programs, facilitate appointments, and assist with navigating barriers to care, including transportation and insurance issues. Providing education on addiction, overdose signs, and naloxone administration, they advocate for low-threshold access to treatment and foster collaborative relationships with patients, families, and community providers. Responsibilities include motivational interviewing, developing treatment and discharge plans, maintaining detailed documentation in EMR, and ensuring continuity of care. They also participate in interdisciplinary teams, respond to patient needs, and support recovery follow-up to promote ongoing engagement. Qualifications include certification as a CPRS, with experience in working with patients with co-occurring disorders. The role requires compassionate communication, cultural sensitivity, and knowledge of addiction and treatment options.
