Outpatient Navigator

For over 60 years, Calvary Healing Center has provided a full continuum of care, specializing in addiction treatment for adults across the Phoenix area and nationwide. Calvary’s proven treatment programs address the practical, physical, and spiritual aspects of addiction for lasting recovery. Our flagship location in Phoenix offers on-site detoxification and inpatient programs, a partial hospitalization program (PHP), and an intensive outpatient program (IOP). We also have satellite clinics offering Intensive Outpatient Programs in the West Valley region and Metropolitan Phoenix. Through our programs, we offer holistic healing options such as yoga, art workshops, and music.

About the Role

The Outpatient Navigator works under the guidance of the Clinical Director to assist with the overall internal growth of the outpatient programs. This role identifies, educates, and facilitates individuals transitioning from the inpatient unit to outpatient programs while improving patient flow and access to outpatient care.

Responsibilities

• Assist in the internal growth of outpatient programs.
• Identify, educate, and facilitate individuals transitioning from inpatient to outpatient programs.
• Improve overall patient flow and access to outpatient care.

Qualifications

• High School Diploma or GED.
• Bachelor’s degree required; Master’s degree preferred.
• Minimum of 2 years of experience in case management-related work.
• Associate’s degree with at least 6 months of behavioral health experience may be considered.
• Must be at least 21 years of age.
• Valid Arizona Fingerprint Clearance Card or ability to obtain within 30 days of hire.
• Completion of BLS and First Aid Training within 30 days of employment.
• Completion of Handle With Care Training within 30 days of employment.
