Responsibilities

• Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Hunterdon, Mercer, Somerset and Warren Counties.
• Provides and coordinates substance abuse services in-home.
• Engages in case management and case documentation.
• Attends and participates in case conferences with Child Protection and Permanency (CP&P) and other providers.
• May work remotely after a 3-month probationary period.

Requirements

• Bachelor’s degree in a behavioral health field and (2) two years related experience.
• Must hold a CADC or LCAD certification.
• Rotating 24/7 coverage required.
• Flexible schedule including availability on nights and weekends.
• Driver’s license needed with an acceptable driving history.

Pay

Salary: $54,325 per year plus Sign On Bonus - $2,500
