Stroke Program Nurse Coordinator, Full Time, 8am-4:30pm, Chestnut Hill Hospital

About the role

Facilitates comprehensive care for stroke patients by coordinating the continuity of care from admission to discharge through collaboration with interdisciplinary team members.

Responsibilities

• Facilitates the implementation of the Stroke Care Guidelines and other evidence/research-based practice.
• Ensures that Hospital's Stroke Program incorporates the recommendations for Primary Stroke Centers published by the Brain Attack Coalition and the American Stroke Association.

Requirements

• Bachelor's Degree in Nursing
• Master's Degree in Nursing preferred
• 3 years experience in adult health with a focus on care of the stroke patient throughout the stroke continuum required.
• Experience in a Clinical Leadership Role preferred

Qualifications

• PA Registered Nurse License Required
• Basic Life Support Required
• Multi State Compact RN License Required or eligible
