About the role
The Stroke Navigator–RN provides expert nursing leadership for stroke patients across the continuum of care, ensuring adherence to evidence-based practice and regulatory requirements.
Responsibilities
- Performs advanced neurologic assessment
- Supports timely clinical decision-making
- Promotes high-quality, coordinated stroke care in alignment with national standards and certification expectations
- Led stroke quality monitoring activities, including chart review, data abstraction, documentation audits, and performance tracking aligned with Get With The Guidelines (GWTG) measures
- Identifies compliance gaps, provides real-time follow-up, reinforces evidence-based practice, and drives continuous quality improvement initiatives
- Serves as a clinical resource to nursing and provider teams, promoting consistent adherence to stroke standards of care
- Supports stroke program operations, staff education, regulatory readiness, and interdisciplinary collaboration to advance patient outcomes and sustain certification excellence
Requirements
- BSN required; MSN preferred
- Minimum of 2 years of clinical nursing experience, 5 years preferred
- Current NYS RN license required, Basic Life Support (BLS) certification required
Qualifications
- Stroke care, quality improvement, or data abstraction experience preferred
- Strong ability to perform chart review, data abstraction, and performance metric tracking
- Proficiency with electronic medical records and data management tools
- Excellent communication, organization, and interdisciplinary collaboration skills
