Stroke Navigator

About the role

The Stroke Navigator–RN provides expert nursing leadership for stroke patients across the continuum of care, ensuring adherence to evidence-based practice and regulatory requirements.

Responsibilities

• Performs advanced neurologic assessment
• Supports timely clinical decision-making
• Promotes high-quality, coordinated stroke care in alignment with national standards and certification expectations
• Led stroke quality monitoring activities, including chart review, data abstraction, documentation audits, and performance tracking aligned with Get With The Guidelines (GWTG) measures
• Identifies compliance gaps, provides real-time follow-up, reinforces evidence-based practice, and drives continuous quality improvement initiatives
• Serves as a clinical resource to nursing and provider teams, promoting consistent adherence to stroke standards of care
• Supports stroke program operations, staff education, regulatory readiness, and interdisciplinary collaboration to advance patient outcomes and sustain certification excellence

Requirements

• BSN required; MSN preferred
• Minimum of 2 years of clinical nursing experience, 5 years preferred
• Current NYS RN license required, Basic Life Support (BLS) certification required

Qualifications

• Stroke care, quality improvement, or data abstraction experience preferred
• Strong ability to perform chart review, data abstraction, and performance metric tracking
• Proficiency with electronic medical records and data management tools
• Excellent communication, organization, and interdisciplinary collaboration skills
