Duties/Responsibilities

  • Drive Enrollment & Conversion: Act as the primary engine for enrolling new members into the Enhanced Care Management (ECM) and Community Supports (CS) program. You will manage the entire engagement cycle, from initial contact and building rapport to successful onboarding and securing a "win" for the individual and the community.
  • Generate Leads & Build Your Pipeline: Proactively canvass communities-to identify and engage potential members. You are not waiting for leads; you are on the front lines creating them.
  • Qualify Prospects with Precision: Master the DHCS eligibility criteria to quickly and accurately assess if an individual qualifies for our life-changing Enhanced Care Management and Community Support Services. This ensures you are investing your time and energy in the right prospects to maximize your impact.
  • Maintain Your CRM & Report on Metrics: Accurately and efficiently track all interactions, conversions, and progress in our systems. Your data is crucial for measuring success, identifying trends, and refining our outreach strategy to exceed targets.
  • Perform other duties as assigned.

Education and Experience

  • Core Foundation: A High School Diploma or GED is required.
  • Proven Track Record: At least one year of experience in a role requiring direct outreach, sales, or community engagement. You know how to connect with people, overcome objections, and guide them through a process.
  • Direct Field Experience: Direct experience working with unhoused or other high-need populations is highly desirable, but not required. You understand the unique challenges and know how to build trust where it's scarce.

Required Skills/Abilities

  • Communication Skills: Excellent verbal and written fluency in English is a must. Ability to communicate with empathy, persuasion, and clarity.
  • Bilingual Advantage: Bilingual skills, particularly in Spanish, are a powerful advantage that will allow you to connect with a broader segment of our community. This is desired, but not required.
  • Resourceful Navigator: A working knowledge of community and county resources is a major asset. You know who to call and where to go to get things done for your members.
  • Tech-Savvy Operator: Proficiency with modern tech tools, including laptops, smartphones, Microsoft Office, and email.
  • Interpersonal Acumen: A street-smart and empathetic individual who can navigate complex social landscapes and build rapport with diverse populations.
  • Sales & Engagement Drive: Possesses the unique blend of sales acumen and relentless drive to meet and exceed enrollment targets.

Physical Requirements

  • This job requires you to be in the community and other community partner sites. 25% of work is often conducted in an office environment and 75% out in the community.
  • Prolonged periods of sitting at an office desk on the computer.
  • Able to lift a minimum of 15 lbs.

Pay Range

$25 USD - $28 USD
