Responsibilities

• Provide personal care and assist with activities of daily living (ADLs) according to the patient’s plan of care (POC).
• Ensure a safe environment for patients.
• Safely ambulate/transfer patients as appropriate.
• Assist with passive and active range of motion (ROM) exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under nurse supervision, assist patients with pre-poured medications that are ordinarily self-administered (Home Care only).
• Perform simple wound care, skin care, and bowel care protocols under nurse direction, as per the POC (where appropriate).
• Recognize and promptly report any changes in the patient’s mental or physical status to the nurse.
• Document activities provided and patient observations in the clinical record.
• Complete charting, timesheets, and mileage logs per agency policy.
• Attend and contribute to patient care conferences, discipline meetings, team meetings, staff meetings, retreats, and in-services.
• Obtain 3 education hours per quarter through in-services.
• Participate in the on-call rotation as scheduled (Home Care only).
• Assist the Team Leader with scheduling, supply organization, answering phones, etc.
• Demonstrate positive customer-service responsiveness.
• Be flexible in meeting patient/family needs, including variable shifts and weekends.
• Participate in chart review quality improvement activities.
• Coordinate and communicate with volunteer staff.
• Provide basic caregiving education to caregivers/family members (e.g., turning, positioning, transfer techniques, changing linens).
• Maintain Transmission-Based Precautions.
• Complete annual competency requirements.

Qualifications

• Must be a state-tested nursing assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications, except where legally required (e.g., license/certification).

Skills

• Proficiency in reading and writing English.
• Ability to work collaboratively in a transdisciplinary team.
• Strong organizational and communication skills.

Conditions of Employment

• Compliance with Annual Flu Shot Policy or ability to provide exemption documentation.
• Active auto insurance policy as a licensed driver in Ohio with proper coverage.
• Active driver’s license.
• Proof of eligibility to work in the United States.
• High level of integrity, ethics, and professionalism.
