Schedule

• Full-time, 4 ten-hour shifts per week
• Weekend requirement: every third weekend

Responsibilities

• Provide personal care and assist with activities of daily living (ADLs) according to the patient’s plan of care (POC).
• Create a safe environment for patients and assist with safe ambulation/transfers as appropriate.
• Perform passive and active range-of-motion (ROM) exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under the supervision of the nurse, assist patients with pre-poured, self-administered medications (Home Care only).
• Perform simple wound care, skin care, and bowel care protocols under the direction of the nurse, as outlined in the plan of care.
• Recognize and promptly report any changes in the patient’s mental or physical status to the nurse.
• Document activities and observations in the clinical record at the time of each visit.
• Complete charting, timesheets, and mileage logs as defined by agency policy and procedures.
• Attend and actively contribute to patient care conferences, team meetings, staff meetings, retreats, and in-services.
• Obtain 3 education hours per quarter through in-services.
• Participate in the on-call rotation as scheduled (Home Care only).
• Assist the Team Leader with designated activities such as scheduling, supply organization, and answering office telephones.
• Demonstrate positive customer-service responsiveness according to agency standards.
• Remain flexible to meet patient/family needs, including variable shifts and weekends.
• Participate in chart review, quality improvement activities, and other assigned meetings.
• Coordinate and communicate with volunteer staff.
• Provide basic caregiving education to family members, such as turning/positioning, transfer techniques, and changing linens.
• Maintain Transmission-Based Precautions.
• Complete annual competency requirements.

Requirements

• Must be able to read and write English proficiently.
• Must be a state-tested nursing assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications except when legal requirements (e.g., license/certification/registration) apply.
• Active auto insurance policy as a licensed driver in the State of Ohio with required coverage levels.
• Active driver’s license.
• Proof of eligibility to work in the United States.
• Compliance with Annual Flu Shot Policy or ability to provide exemption documentation.
• High level of integrity, ethics, and professionalism.
