What You Will Do

The Patient Service Coordinator is a professional whom is directly responsible for scheduling patient visits and needs to field clinicians. The Patient Service Coordinator collaborates with internal and external sources (i.e., field staff, patients, caregivers, referral sources, providers, community sources and Hospice IDG members) to support scheduling needs and continuity of care.

The Patient Service Coordinator is a resource person for all patient scheduling needs and works closely with other members of the Hospice IDG Team to ensure adequate coordination and transition occurs while on hospice services. The Patient Service Coordinator should have extensive knowledge of the regulatory guidelines governing hospice scheduling requirements.

Assists with administrative duties as assigned. Attends and actively participates in IDG and team meetings. Participates in Holiday staffing.

Requirements

  • High School diploma or equivalent
  • Basic knowledge and skills to care for the clinical needs of the population served
  • Professional and efficient verbal and written communication skills
  • Professional interpersonal skills
  • Ability to multi-task, basic organizational and time management skills
  • Basic computer skills
  • Knowledge of general office and administrative practices and procedures
  • Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.)
  • Ability to be open, sensitive, flexible and ecumenical
  • Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously
  • Current Texas state driver’s license
  • Current auto insurance
  • Must have reliable transportation

Preferred Qualifications

  • Some college or business courses
  • Experience with Home Care Home Base
  • Experience with smart phones and smart devices
  • Advanced education or certification in area of specialty
  • Home Care or Hospice experience
  • Experience in a healthcare setting
  • Experience in scheduling
