About the role

Coordinates professional nursing and individualized, family-centered care to patients/families through the use of the nursing process and clinical skills in accordance with standards and hospital policies and procedures. On-call responsibilities vary depending on the department. Varied schedules include weekend and holiday patterns.

Responsibilities

• Conducts individualized age-specific patient/family assessments according to defined unit scope and nursing practice standards, appropriate collaborative problems (CPGs)/nursing diagnoses (NDs), or protocols.
• Admission Assessment:
  • Collects and documents data related to patient’s individualized care.
  • Demonstrates age-specific competency in assessment of patients by obtaining and interpreting information in terms of patient needs, growth and development, and understanding of treatment needed.
  • Assesses patient/family need for education and uses an individualized approach in delivery style.
  • Assesses family expectations for, and involvement in, the assessment and continuous care or treatment of the patient.
  • Identifies nursing diagnoses, collaborative problems, and/or protocols for each patient at the time of admission.
• Shift Assessment/Reassessment:
  • Performs reassessment based on unit standards, CPGs/NDs, protocols, and changes in patient condition.
  • Communicates changes to appropriate persons (MD, Charge RN, family) as indicated.
• Cath Lab:
  • Schedules cardiac catheterization and electrophysiology procedures with patient and family.
  • Works closely with all affected personnel: OR supervisors, OR Manager, HCL staff, and Director of Anesthesia.
  • Obtains permits for catheterization procedures after teaching patient/family about the procedure and related complications.
  • Collects and documents data related to patient’s response to procedure and changes in condition according to hospital policy.
  • Demonstrates age-specific competency in assessment of patients by obtaining and interpreting information in terms of patient needs, growth and development, and understanding of treatment needed.
  • Circulates in procedures following AORN guidelines/standards and assists at bedside in preparing patients for each procedure.
  • Applies critical thinking during catheterization procedures in delivering direct patient care and analyzing response to medications and treatment, prioritizing intervention based on continual assessment.
  • Coordinates with referring physician and other operating physicians, assessing patient/family care issues in scheduling procedures.
  • Accompanies patients/family to room to help facilitate patient flow.
• Develops individualized patient/family plan of care using CPGs/NDs and practice guidelines/protocols to meet identified needs, including care, education, and discharge needs.
• Utilizes assessment data from the healthcare team to individualize appropriate goals and actions for the patient, including collaboration with family and healthcare team.
• Formulates goals and outcomes based on assessment data and revises the plan of care based on patient’s condition, interdisciplinary input, and family needs.
• Demonstrates ability to prioritize patient interventions based on assessments/reassessments.
• Implements and evaluates nursing actions in accordance with the individualized patient/family plan of care and patient condition/needs.
• Oversees that the nursing and interdisciplinary plan of care/goals are coordinated, implemented, evaluated, and revised as needed.
• Identifies the need for interdisciplinary involvement/outside resources and follows through.
• Uses appropriate resources (CNS/Educator, Director, MD, etc.) to ensure clinical outcomes are achieved.
• Incorporates critical thinking into the delivery of clinical skills and analyzing patient response to orders, tests, labs, medications, and treatments.
• Demonstrates clinical competence in administering interventions according to identified standards and age-specific needs.
• Provides and/or coordinates patient/family education according to identified care needs, considering assessment of readiness to learn.
• Documentation reflects accurate assessment/evaluation, modification, and patient’s response to therapy, changes in condition, and therapeutic interventions/procedures.
• Entries in the medical record follow hospital policy (e.g., date, time, use of appropriate abbreviations, and legibility).
• Documentation is completed within established timeframes, including Admission Assessment form, daily Patient Flowsheet, Peripheral IV Flowsheet, Family Education Record, and Interdisciplinary Goals.
• Reviews and updates computerized care plan to accurately reflect the plan of care.
• Completes computerized documentation within established timeframes, including Medication Administration Record, Allergies, Patient Information, etc.
• Completes and verifies accurate order entry in the computerized care plan.
• Ensures that discharge instructions clearly state patient care needs after discharge.
• Assists FCC Coordinator in arranging and coordinating appointments prior to the delivery process.
• Accepts assignment to mentor student nurse observation experience in unit.
• Accepts assignments to share/teach knowledge and provide clinical support as a relief preceptor for new staff.
• Professionally seeks opportunities to help teammates during shift as needed.
• Accepts and completes other assigned tasks in a professional manner.
• Regular attendance at work is an essential function of the job.

Requirements

• Associate's Degree from an accredited school of nursing or Bachelor's Degree from an accredited school of nursing.
• Minimum 1 year of RN experience required.
• Experience with pediatric patients preferred.
• Previous experience with EPIC preferred.
• Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing skills.

Qualifications

• Registered Nurse - State Licensure and/or Compact State Licensure current and valid in the state of Nebraska required upon hire.
• Basic Life Support from the American Heart Association required upon hire.
• Pediatric Advanced Life Support (except NICU) required within 180 days.
• Neonatal Resuscitation Program (NICU only) required within 180 days.
• Advanced Cardiac Life Support (Cath Lab, CARES, and ED only) required within 180 days.
• Emergency Nursing Pediatric Course (ED only) required within 1 year.
• Trauma Nursing Core Course (ED only) required within 1 year or Advanced Trauma Care for Nurses (ED only) required within 1 year.
• For RNs working in Radiology Nursing, training in all areas of Radiology Nursing and completion of all required competencies required.

Schedule

0900-2130 - Lifestyle 9:3 - 36 hours per week. Varied schedules include weekend and holiday patterns.
