About the role

Community Healthcare Network is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually. This position is for our Washington Heights Health Center located at 511 W. 157th Street New York, NY 10032.

Responsibilities

• Providing evidence-based Primary Care to patients
• Integral member of the collaborative care team
• Providing care of all ages in Family Medicine
• Prenatal care and Women's Health procedures (strong interest is ideal)

Requirements

• Valid N.Y.S. License to deliver medical care
• At least one (1) year of post-graduate/residency experience delivering care in a Primary Care setting
