Staff Nurse (LPN)

Brief Description POSITION SUMMARY: The Licensed Practical Nurse (LPN) - Population Health supports the delivery of proactive, patient-centered care by assisting with care coordination, chronic disease management, preventive health initiatives, and quality improvement activities. Working as part of a multidisciplinary team, the LPN engages patients through outreach, education, and follow-up to improve health outcomes, close care gaps, and promote adherence to treatment plans. The LPN plays a key role in supporting value-based care initiatives, preventive screenings, care transitions, and ongoing management of patients with chronic conditions. Requirements HS/GED requiredCurrent LPN license in good standing with the Florida Board of Nursing 1+ years Clinical experience in an ambulatory care setting preferred Current CPR certification requiredExcellent listening and interpersonal skillsMust be flexible, resourceful, and able to troubleshootMust be able to handle multiple tasks simultaneously and set prioritiesPride in the job you do and the image you present to our patients & visitorsA positive can-do attitudeBilingual (Spanish/English) a plus Summary POSITION SUMMARY: The Staff Nurse LPN ensures services are administered utilizing professional standards with a strong focus on quality, timely care, and exceptional customer service under the direction of the medical provider.
